The wellbeing of our residents is our top priority, and each resident is assigned a Primary Care Nurse, who will discuss and assess with them all medical and wellbeing needs. Following on from this, a person-centred plan is created, and regularly reviewed. This Primary Care Nurse is responsible for ensuring a continuum of care is carried out effectively.

Hazelwell Care Home is a purpose-built, luxury home that is able to cater to a wide range of residents and support a wide range of medical needs. This year, we have proudly unveiled a brand new additional unit, ‘Gayton Nursing Community’. Named after our local neighbourhood of Gayton, we have created this community within our home to support residents with nursing needs, who have a variety of physical conditions.
